PrimeHub Enrollment Agreement
Welcome to PrimeHub! We look forward to a healthy and happy relationship with your family. This Enrollment Agreement outlines our tuition and other policies, created to help ensure the smooth operation and safety of our program.
In consideration of the enrollment of the above-named student (the "student") by Prime Foundation Montessori ("PrimeHub") for your chosen program and intending to be legally bound, we, the parents or legal guardians of the student, jointly and severally, understand and agree to the terms and conditions of this Enrollment Agreement (the "Agreement"). By signing this Agreement, we accept the place Prime Foundation Montessori has reserved for the student for our chosen PrimeHub program . When the pronouns "we," "our" or "your" are used in this Agreement, they refer, collectively, to all the parents or guardians who sign this Agreement. Enrollment in PrimeHub for our chosen PrimeHub program , unless terminated sooner under the terms of this Agreement. Enrollment in any prior year at PrimeHub does not guarantee enrollment for any subsequent school year.
Tuition And Fees
Bills & Payments: Prime is offering three payment plans to allow parents more flexibility in paying tuition.
Prorated Tuition and Joining Mid-Year: For students starting mid-year, tuition is prorated starting from lesson #3. The registration fee, tuition for the partial month and the following month is due before the first lesson. All other tuition will be charged on the 1st of the month based on the payment plan selected. For example, a student that joins on November 15th will be charged for the two remaining lessons in November along with the 4 lessons in December. Their next payment will be due on December 1st and cover will January. No refunds are made for students who miss classes throughout the year.
Registration Fee: Students enrolled for the fall and spring semesters are required to pay a $100 non-refundable annual registration fee. This fee covers the cost of copies and supplies and reserves your place in class. A placement in class is not guaranteed until the registration fee is received. The fee is reduced to $80 for students registering after January 1st and to $40 after April 1st.
Summer School applicants are not required to pay a registration fee.
Late Fee: All tuition is due on the 1st day of the month. A $35 late payment fee is automatically applied to your account on the 10th of the month.
Advance payment and Installment Plan Fee: Regardless on the payment plan all Tuition is charged one month ahead [for example October tuition will be charged no later than September 1st and September tuition is due on August 1st]. A 9-payment installment plan is available for parents but has an additional one-time fee of $46 per family.
Tuition: The tuition below is reserved for those families living in the community of our school. Your actual tuition may differ based on your residence, as "PrimeHub " is a community initiative program.

Placement, Withdrawal, and Cancellation Policy
PrimeHub is committed to making sure the student has an enriching educational experience. Sometimes PrimeHub may need to work together with you to find the right class options for the student. Please contact PrimeHub early if you have concerns about the student's progress or are in any way dissatisfied. PrimeHub assigns the Student to the grade level and teacher it deems to be the most appropriate for the student's academic level. Teachers and courses are subject to change, and adjustments may be made without prior notice by PrimeHub If you wish to withdraw the student based on class or teacher assignment, the cancellation policy outlined below applies.
• For New students
If a written cancellation notice is received by PrimeHub anytime before the 3rd lesson, the first 2 lessons will be considered a trial period, and all tuition will be fully refunded (not including the registration fee). After the 3rd lesson and before the 20th lesson, cancellations are accepted anytime with a 1 month (4 lessons) written notice. After Lesson 20, all new students are considered continuing and should follow the cancellation policy of a continuing student. If a student attended the PrimeHub Summer Program, this student is considered continuing and should follow the policy of a continuing student. PrimeHub should be notified about a cancellation via email.
• For continuing students, a refund for Fall Semester (September - January) will be processed as follows, only after PrimeHub receives a written cancellation notification:
o 100% of tuition is refunded if the cancellation notification is received before the student's first lesson.
o If the cancellation notification is received before the second lesson the charges will be applied for 4 lessons and rest of the tuition will be refunded.
o If the cancellation notification is received before the third lesson the charges will be applied for 8 lessons and rest of the tuition will be refunded.
o If the cancellation notification is received before the fourth lesson the charges will be applied for 12 lessons and rest of the tuition will be refunded.
o If the cancellation notification is received before the sixth lesson the charges will be applied for 16 lessons and rest of the tuition will be refunded.
o There are NO REFUNDS for the Fall semester after the 6th lesson.
o For students enrolling into full-year classes, - 1-month cancellation notice (4 lessons) is required for all students, whether new or continuing.
• For continuing students withdrawing during the Spring Semester (February - June), a full refund will be issued if a written withdrawal notice is received by lesson 20. After lesson 20, NO refunds will be issued for the Spring Semester.
• All persons signing this Agreement are jointly and severally responsible for our performance hereunder, including all payments, and shall be responsible to reimburse PrimeHub for all expenses associated with collecting tuition, fees, and other costs, including reasonable attorneys' fees and costs.
Student Health Policies
• Prior to enrollment, any allergies or medical conditions that may affect the student's experience or safety should be communicated to the branch's principal or administrative staff. Student health information is maintained privately, with only those staff informed on a need-to-know basis.
• We understand that PrimeHub does not employ a school nurse or any other licensed, medical professionals and that its employees and staff may not be trained on how to administer any medication, including emergency medication (Epi-Pens).
• We understand that if the student must take medication during the time the student is at a PrimeHub location, the student must be able to self-administer the medication, or we must be present to assist. We understand that we may not rely on PrimeHub employees or staff to administer medication to the student.
• Each class has a 10-minute break period during which the student may eat snacks brought from home. We agree not to pack any snacks that contain nuts. PrimeHub locations are "nut-aware," but PrimeHub does not guarantee a nut-free environment.
• We understand that in case of an emergency allergic reaction, PrimeHub will administer an Epi-Pen only if we sign the EPIPEN Authorization and Waiver of Liability.
• In the event of serious injury or medical emergency, we authorize PrimeHub, its employees, and agents to act on our behalf, in our absence, and in the event we cannot be reached, to arrange for and give consent to medical treatment for the student. This treatment may include diagnostic, medical-testing, therapeutic, and/or operative procedures as may be deemed necessary for the student. We understand that no operative procedure will be performed on the student, except in an emergency, without a reasonable effort on the part of PrimeHub to contact us. In the event of a minor injury or illness, including, but not limited to, a small cut, a cold, or an ordinary infection, we authorize PrimeHub to provide first aid and/or administer over-the-counter medications. We understand that we may or may not be notified of these minor events.
Student Media Release
• PrimeHub's ability to portray its program accurately and vibrantly depends on families' support of PrimeHub's use of images of students and the students' work. It is PrimeHub's general practice not to name students when including them in any form of media (print or electronic); however, on occasion, students will participate in promotional or academic projects in which it makes sense for students to introduce themselves or otherwise be named. It is PrimeHub's policy not to use student names without first obtaining the permission of the student's parents or guardians.
• By signing this Agreement (and unless we check the "opt out" box below), we authorize PrimeHub, its successors and assigns, and those acting within its permission and upon its authority, to use the student's name, photographic image (including, but not limited to, portrait, picture, video, or other reproductions), audio recordings of the student's voice, video recordings of the student, and likeness, in written or in electronic format, and/or reproductions of the student's work (collectively referred to herein as "Student Media Information") in PrimeHub's publications, marketing and promotional materials, website, press releases, and/or advertising media. We waive the right to inspect or approve the finished product and acknowledge PrimeHub's right to crop or alter any photographic image of the student at its discretion. We authorize PrimeHub to use the Student Media Information on more than one occasion, without limitation to the number of times it is used, in perpetuity; and to reproduce the Student Media Information or cause it to be reproduced. We shall not be entitled to receive any compensation for such use, and hereby release PrimeHub, its successors and assigns, and those acting within its permission and upon its authority, from any liability, responsibility, or claim that may arise by reason of any exercise of the authority granted above.
Additional Policies
Parent Handbook. We acknowledge that we have received a copy of the PrimeHub Parent Handbook ("Parent Handbook") and we agree to abide by the policies contained in the Parent Handbook, as they may be amended from time to time. PrimeHub may dismiss the student if he/she or we violate PrimeHub's policies, rules, or regulations. PrimeHub may also dismiss the student whenever, in the exercise of PrimeHub's professional judgment, it may be appropriate to do so, including as a result of the student's behavior or academic performance, failing to meet PrimeHub's standards as described in the Parent Handbook or when the student's conduct, our own conduct, or the conduct of anyone affiliated with the student is not in keeping with RSM's standards. If the student is dismissed pursuant to this paragraph, RSM reserves the right not to issue a tuition refund. RSM, in its sole discretion, may amend any of its policies at any time and without notice.
Disclosure. It is in the best interest of the student for RSM to be aware of the student's medical, disciplinary, and emotional health history (the "Student History") if such information would affect the student's experience at RSM. If at any time the student engages in any behavior that would preclude admission under RSM's policies or acceptance criteria, then RSM may dismiss our student or revoke acceptance, as the case may be. Those circumstances may include our student engaging in criminal or violent acts or becoming academically ineligible. RSM may also dismiss the student whenever, in the exercise of its professional judgment, RSM determines that our own conduct, or the conduct of anyone affiliated with the student, is not in keeping with RSM's standards.
Student Access. In the event of divorce or separation, it is RSM's policy to provide the custodial parent, the non¬custodial parent, and stepparents equal access to all the official records and reports regarding the student as well as all RSM announcements and related information. Moreover, each is granted access to the student, to the teachers, and to administrators. Such access is provided without notification to other parties and regardless of the party or parties named who claim financial responsibility. Exceptions to this policy are undertaken only when necessary to comply with applicable law and/or when so ordered in writing by a court of law. We agree to provide RSM with the names of all parent(s) and guardians with custodial authority over the student and to provide full legal documentation to verify that status if requested by RSM. Any additionally named custodial parent or guardian(s) will be required to sign an acknowledgement that they accept the provisions of this Agreement and the contents of the Parent Handbook.
Use of RSM Logo. We will not use RSM's trademarks, logos, or images without RSM's prior written authorization, which RSM may withhold in its discretion.
Force Majeure. We understand and agree that the duties and obligations of RSM under this Agreement may be modified or suspended immediately, without notice, because of force majeure events, including, but not limited to, fire, acts of God, war, governmental action, terrorism, epidemic, pandemic, or any other event beyond RSM's control. If such an event occurs, RSM's duties and obligations under this Agreement may be modified, suspended, or postponed until such a time as RSM, in its sole discretion, may safely resume operations. In the unfortunate event of a forced school closing, RSM may continue to operate on a distance-learning basis for a period of time RSM deems appropriate, or RSM may extend the school year, schedule classes on weekends or during vacations, if RSM is able to reopen, in order to make up for a closure. We acknowledge and agree that the sole remedy for a force majeure event is future service delivery and not a tuition refund.
Entire Agreement. This Agreement represents the entire agreement between the parties with regard to the matters herein and supersedes any other agreement or understandings, whether written or oral, and cannot be changed unless agreed upon and signed by an authorized representative of RSM in writing. Any waiver of any obligation of either party to the Agreement by the other party shall not be construed to be a waiver of any other obligations, or the same obligation, on a future occasion.
Severability. The terms of this Agreement are independent of each other. If a term, in the Agreement is deemed unenforceable by a court, the remaining terms shall remain in full force and effect. Multiple copies of this Agreement may be signed, all of which constitute one and the same agreement.
Jurisdiction. This Agreement shall be interpreted under California law without considering conflict of laws principles. The jurisdiction and venue for any disputes arising from this Agreement or the student's enrollment in RSM shall exclusively be in the courts of California or in a court located in the state in which the student is enrolled in RSM and each party irrevocably submits to the exclusive jurisdiction of such courts in any suit, action, or proceeding. The parties irrevocably and unconditionally waive any objection to venue of any suit, action, or proceeding in such courts and irrevocably waive and agree not to plead or claim in any such court that any such suit, action, or proceeding brought in any such court has been brought in an inconvenient forum.

By checking the box below, we agree that this Agreement is valid and will be given the same legal effect as a written and signed agreement. We further acknowledge and agree that no certification authority or other third-party verification is necessary to validate our electronic signatures, and that the lack of such certification or third-party verification will not in any way affect the enforceability of our electronic signatures of this binding Agreement between RSM and us. Our consent to the electronic signature below applies only to this Agreement and does not apply to any separate transactions or communications with RSM. We may request a paper copy of my completed and binding.
